International Student Health Insurance
Notice: Student Health Services only accepts the University sponsored insurance for payment at this time.
Students with insurance from other carriers, may request a completed claim form (HCFA 1500) to submit to their insurance provider for reimbursement of covered services. Students enrolled in the FAMU SHIP, will not need to complete a reimbursement form.
International students must present proof of insurance coverage along with the required Health History and Immunization form to the Student Health Services.
