Exchange Students


WELCOME to Florida A&M University (FAMU)! We are glad you have chosen us for next Spring 2019.


The following forms are required to start your process with us. Please complete ALL forms required, and submit them to   Please READ carefully and all the email.


1.     Non-Degree Seeking Student Application Form (form attached; please complete ALL information required)

2.     Request for Certificate of Eligibility Form DS-2019 (form attached; ALL information required, except for DEPARTMENT INFORMATION: FAMU will complete it):



a.     The DS-2019 Form is the document that allows you to apply for the J-1 Visa as an Exchange Student, Intern, or Trainee in the USA. The DS-2019 form is also called “Certificate of Eligibility”. It is issued to you by CICD, a US Department of State-designated sponsor organization.

o    Please complete ALL spaces required in the Financial Information. In total of at least $15,000 U.S. dollars.

o    Submit COPY OF BANK STATEMENTS to show you have enough funds for living expenses.

o    If the student will receive a scholarship; please submit COPY OF THE LETTER OF SCHOLARSHIP showing the exact amount of funds you will be receiving and the duration of the scholarship.

o    NO CHECKS are allowed as part of the SOURCE OF FUNDING.

3.     FAMU Health History and Immunization Form (form attached; please read carefully and follow all the instructions in the form; complete ALL information required *read NOTE below).

a.     Student Health Services provides a wide range of immunizations to enrolled FAMU students.  Many extremely valuable vaccines are available to help prevent disease. For more information, please click here:

·         Provide documentation of two (2) MMR immunizations

·         Provide documentation of Meningitis and Hepatitis B immunization or signed waiver

4.     FAMU Academic Advisement Form: (form attached; complete ALL information required and include Course Prefix #, Course Title and Credit Hours, Min. 12 credits for semester or four classes). The Academic Advisor must approve the form previous to your arrival on campus and start classes on January 7, 2019. Please contact your Academic Advisor for more information.

5.     Copy of University Official Transcripts



6.     Copy of your passport biographical page


7.     Copy of English Proficiency: please submit copy of scores resulting from the TOEFL, (TOEFL score requirements are: (computer-based 173-; Internet-based 80) or IELTS 6.



Once you send ALL documents required, the Register Office can process your request. Once you receive the FAMU Acceptance Letter, Ms. Coppin, our International Students and Scholars Director, will start working on your DS-2019 Form.


Remember the following information: 


a.     J1 Visa Insurance Requirements:

As part of the J1 visa requirements, you (and any family members traveling with you) will be required by the Department of State to maintain adequate health insurance coverage for the full duration of your J-1 program, that meets the following main requirements:


Medical benefits of at least $100,000 per accident or illness

• Repatriation of remains in the amount of $25,000

• Expenses associated with the medical evacuation of the J1 visitor to his or her home country in the amount of $50,000

• A deductible not to exceed $500 per accident or illness


Upon arrival to Florida A&M University, please be prepared to provide the required evidence of health insurance.  To learn more about this requirement, go to the following link: and scroll down to the bottom of the page.



b.     *NOTE: Health Insurance:

International students must present proof of insurance coverage along with the required Health History and Immunization form to the Student Health Services. Policy information must be presented in English and meet the benefit requirements established by the Florida Board of Governors 

Students, who have not yet purchased an appropriate insurance coverage, may obtain information from the Office of Student Health Services on the health insurance policies from a few insurance companies that have been reviewed by the University and that meet the state requirements.

**NOTE: Student Health Services only accepts the University sponsored insurance for payment at this time. Students with insurance from other carriers, may request a completed claim form (HCFA 1500) to submit to their insurance provider for reimbursement of covered services. Students enrolled in the FAMU SHIP, will not need to complete a reimbursement form.
